I think once it goes to true collection agency you are SOL for negotiating - take the payment plan option if you can and negotiate a really low monthly payment. If it is still coming from the actual medical practice, then definitely try to ensure the bill is correct and they can't discount it any (I'm assuming you don't really qualify for sliding scale by being poor, but if so that's another option).
